Instructions for signing up for secure email to Amherst Family Practice through Medem

The process of signing up for secure email with Amherst Family Practice is not difficult, but a quick tour of it may be helpful for expediting your access to this feature. You may wish to print this page to have available when you navigate through the menus and pages you will see.

1. When you click on "Sign up for Medem email" below, you will first see our home page in the Medem system. If you have not used this before, click on "New User" and you will be given an opportunity to indicate your own physician: Dr. Anne Weaver or Dr. Kinga Pluta.

2. When you have made this choice, you will be taken to the page where you register for email. Here you will choose a "User ID" and a Password. BE SURE THAT YOU WRITE THESE DOWN SO YOU WILL REMEMBER THEM. You then will answer a number of questions for identification. Those items with an asterisk (*) are required for proper identification and, in the case of your email address, for a way to respond to your inquiries. When you complete filling in this information, press "Send Registration." If all is correctly done, you will see a confirmation and welcome from your doctor indicating that you are now registered to use the Medem email system. Press "Return to your health care provider's Home page."

3. Now you can "Log in" to use the email functions. Enter the User ID and Password that you chose previously. Also, you must "check" the box by the sentence that begins "I understand that this practice...." Press "Continue." If you had not checked that box, you will be instructed to return to this page and check it.

4. You are now logged in to email and are returned to your doctor's Medem home page.

5. To send a message (or later to read any messages that you have received), click on "Send or Check Messages."

6. At this point, you get a page entitled "MEDEM SECURE MESSAGING AND ONLINE CONSULTATION TERMS OF SERVICE" with a detailed statement regarding the legal terms and ramifications of using Medem's email system. You should read this and indicate if you accept or decline the terms. If you decline the terms, you will not be able to use the email service.

REGARDING PAYMENT FOR EMAIL CONSULTATIONS WITH THIS OFFICE, Amherst Family Practice is not participating in that function and we do not charge for your communication by email. Email should not be used for urgent matters or detailed medical consultations.

Once you accept the terms, you will not need to repeat this process. You can always review the Terms of Service by clicking on the link at the right side of the page.

7. You are now ready to use the email system. You should choose the type of message you want to send (General Message, Prescription Renewal Request, or Appointment Request) and an appropriate page will come up on which you can write your message. Alternatively, if you have messages already in your mailbox, you can read these by clicking on those messages listed below on the page.

8. When you are finished, click on "Log off."
